Health plans offered through Benefit Administrators requires you to submit detailed health history information as part of their medical underwriting process. The medical health history information is collected on behalf of the insurance company or health plan. We need you to complete the medical health history section of the health plan's application in order for the carrier to underwrite your application. Benefit Administrators does not store any personal health information on its servers and promotes the use of completing the actual health plan's application for submission. Whenever there is an online application provided by Benefit Administrators, this is a direct application submission to the carrier and is encrypted with the Secure Socket Layer (SSL) system. All of your information is kept confidential between you, Benefit Administrators and the insurance company/health plan you select. None of the information provided is accessible to anyone else. Once your application has been submitted to the insurance company/health plan and you wish to correct or update your information, you can either send the information to Benefit Administrators for submission or you may send it in directly to the insurance company/health plan. |
